Please note the additional documentation needed to complete the application process for employment at ProMed Staffing, Inc.
- Completed application & resume
- Current License or Certification
- Current CPR (must be renewed annually for agency personnel per state law)
- Current Special Certifications (i.e. PALS, NRP, ACLS, etc…)
- Completed Skills Check List
- Completed Competency Exam
- Completed Physical or Statement of Good Health (annually)
- Varicella, Rubella, Rubeola History or Immunizations (some facilities may require Titers)
- Hepatitis B: Proof of Immunization or Titer or Signed Declination
- TB Test Results (annually); if positive, must have Chest X-Ray every 5 years & complete yearly TB questionnaire
- I-9 Documentation: Valid ID with Social Security Card, Birth Certificate or Passport
- W-4 Documentation
- Signed Reference Forms
- Negative Drug Screen
- State Background Check
- Minimum One Year Experience
- Age Specific, OSHA, Infection Control and Fire Safety Competencies
- Other Forms: Paycheck Preference, Direct Deposit, Timecard
